While businesses begin to communicate more with other businesses around the world, it is equally important to remember the advantages of becoming diverse domestically.†¦show more content†¦So putting more people of color on the sales floor, and in executive positions, is really a no-brainer in any company†. Customers that can relate to representatives are more inclined to do business with that particular company. To give an example, Toyota works to find highly capable minority employees by going outside of the automobile industry to get someone who is energetic and shows potential in becoming an automobile dealer. After they find the right person, Toyota then puts that person through a lot of training, giving them the tools and resources they need to become successful in the industry (Prenhall.com). Toyota’s ability to look for personal traits instead of a person’s years of experience in the field has given them the reputation of being minority friendly. This has potentially given them an advantage over other automakers, also giving them the potential to out-perform Wall Street. As the business environments continue to increase in the level of globalization, companies find it rewarding to continue to invest in the development and expansion of their own diverse teams. They do this by following the guidelines and strategies outlined by Daniels earlier. The Romantic Period Of William Wordsworth, Samuel Taylor...

The Romantic period was primarily an intellectual movement filled with and moved by art, and literature. The era is seen by many as an outbreak or revolt against the â€Å"norms,† not only that but many see the movement as one of the main reactions to the rationalization of both science and nature as well, the era that some would refer to as the Enlightenment. When thought of or talked about today Romanticism is more commonly associated with liberalism and radicalism. The movement was based on the idea that intense emotion could actually be used as an actual and valid source, it also confronted the sublimity of nature and many, if not all, of its picturesque qualities. This period was home to many great writers, writers such as Percy Bysshe Shelley, William Wordsworth, Samuel Taylor Coleridge, and Mary Shelley. One can imagine that among the many topics that preoccupied the many different works and ideas of these Romantic poets and writers was the Romantic conception of sublim e, or idea of being able to connect to one’s own experiences of awe and other emotions like those of terror or even danger. Where had the week gone? I was driving Adrian to pick up his car in the morning, which hopefully wouldn’t take a long time. â€Å"Could we meet at noon? I’ve got an errand to run.† â€Å"I suppose so,† said Ms. Terwilliger, with some reluctance. â€Å"Meet me at my place, and then we’ll go out to Lone Rock Park.† I was about to lie back on my bed and froze. â€Å"Why do we have to go out to the middle of the desert?† Lone Rock Park was remote and rarely saw many tourists. I hadn’t forgotten how terrifying it was the last time she’d brought me out into the wilderness. At least this time we’d be in daylight. â€Å"Well, we can hardly practice on school grounds,† she pointed out â€Å"True. . . .† â€Å"Bring your book, and the components you’ve been working on.† We disconnected, and I jotted out a quick text to Adrian: Need to be fast tomorrow. Meeting Ms. T at 12. His response wasn’t entirely unexpected: Why? Adrian naturally needed to know everything that was going on in my life. I texted back that Ms. Terwilliger wanted to work on magical protection. This time, he did surprise me: Can I watch? Wanna know how she’s protecting you. Wow, Adrian actually asked? He had a history of simply inviting himself along on outings. I hesitated, still confused after our heated moment at the sorority. He’d never mentioned it again, though, and his concern now touched me. I texted back that he could come along and was rewarded with a smiley face. I didn’t entirely know what to wear to â€Å"magical training,† so I opted for comfortable layers the next morning. Adrian gave me a once-over when he got into Latte. â€Å"Casual mode, huh? Haven’t seen that since the Wolfe days.† â€Å"I don’t know what she has in mind,† I explained, doing a U-turn on his street. â€Å"Figured this was best.† â€Å"You could have worn your AYE shirt.† â€Å"Wouldn’t want to get it dirty,† I said, grinning. That was partially true. I still thought the fiery heart he’d painted was exquisite. But each time I looked at the shirt, too many memories seized me. What had I been thinking? That was a question I’d asked myself a hundred times, and every answer I came up with sounded fake. My preferred theory was that I’d simply been caught up in how serious Adrian had been about his art, how the emotion and passion had seized hold of him. Girls liked artists just as much as bad boys, right? Even now, something stirred in my chest when I thought about the enraptured look on his face. I loved that he possessed something so powerful in him. But, as I told myself constantly, that was no excuse for climbing all over him and letting him kiss me – on my neck. I’d bought and downloaded the â€Å"bad boy† book online, but it had been completely useless in advising me. I finally decided the best way – if not the healthiest one – was to act like the moment had never happened. That didn’t mean I forgot it. In fact, as I sat beside him in the car, I had a difficult time not thinking about how it had felt to be pressed up against him. Or how his fingers had felt entangled in my hair. Or how his lips had – Sydney! Stop. Think of something else. Conjugate Latin verbs. Recite the periodic table. None of those did any good. To Adrian’s credit, he continued to withhold any commentary about that night. Finally, I found distraction in telling him about my trip to San Bernardino. Rehashing the conspiracy, rebel groups, and break-ins pretty much killed any passionate feelings I still had. Adrian didn’t like the idea of Alchemists working with Warriors or of the tattoo controlling me. But he also didn’t like me walking into danger. I tried to downplay the near impossibility of breaking into the St. Louis facility, but he clearly didn’t believe me. Ms. Terwilliger texted me twice not to be late to our meeting. I kept an eye on my watch, but the care of a Mustang was not something I took lightly, and I had to take my time at the mechanic’s shop to make sure the Mustang was in pristine condition. Adrian had wanted to go with basic tires, but I’d urged him to upgrade, convincing him the extra cost would be worth it. I cast a large spell this morning that won’t last forever. The countdown is on.† She wouldn’t say any more until we reached the park, and that silence unnerved me. It gave me the opportunity to imagine all sorts of frightening outcomes. And although I trusted her, I suddenly felt relieved that Adrian was along as a chaperone. Although it wasn’t the busiest place, Lone Rock Park still had the occasional hiker. Ms. Terwilliger – who was actually in hiking boots – set off across the rocky terrain, searching for a suitably remote space to do whatever it was she had in mind. A few stratified rock formations dotted the landscape, but I couldn’t really appreciate their beauty. Mostly I was aware that we were out here when the sun was at its fiercest. Even if it was almost winter, we’d still be feeling the heat. I glanced over at Adrian as we walked and found him already looking at me. From his jacket pocket, he produced a bottle of sunscreen. â€Å"I knew you’d ask. I’m nearly as prepared as you are.† â€Å"Nearly,† I said. He’d done it again, anticipating my thoughts. For half a heartbeat, I pretended it was just the two of us out on a pleasant afternoon hike. It seemed like most of the time we spent together was on some urgent mission. How nice would it be to just hang out without the weight of the world on us? Ms. Terwilliger soon brought us back to our grim reality. â€Å"This should do,† she said, surveying the land around her. She had managed to find one of the most desolate areas in the park. I wouldn’t have been surprised to see vultures circling overhead. â€Å"Did you bring what I asked for?† â€Å"Yes, ma’am.† I knelt on the ground and rifled through my bag. In it was the spell book, along with some herbal and liquid compounds I’d mixed up at her request. â€Å"Take out the fireball kindling,† she instructed. Adrian’s eyes went wide. â€Å"Did you just say ‘fireball’? That’s badass.† â€Å"You see fire all the time,† I reminded him. â€Å"From Moroi who can wield it.† â€Å"Yeah, but I’ve never seen a human do anything like that. I’ve never seen you do anything like that.† I wished he didn’t look so awestruck because it kind of drove home the severity of what we were about to attempt. I would’ve felt better if he’d treated it like it was no big deal. But this spell? Yeah, it was kind of a big deal. I’d once performed another spell that involved throwing a painstakingly made amulet and reciting words that made it burst into flames. That one had a huge physical component, however. This spell was another of those mental ones and essentially involved summoning fire out of thin air. The kindling Ms. Terwilliger had referred to was a small drawstring bag filled with ashes made from burnt yew bark. She took the bag from me and examined its contents, murmuring in approval. â€Å"Yes, yes. Very nice. Excellent consistency. You burned it for exactly the right amount of time.† She handed the bag back. â€Å"Now, eventually you won’t need this. That’s what makes this spell so powerful. It can be performed very quickly, with very little preparation. But you have to practice first before you can reach that point.† I nodded along and tried to stay in student mode. So far, what she was saying was similar to what the book had described. If I thought of all this as a classroom exercise, it was much less daunting. Not really scary at all. Ms. Terwilliger tilted her head and looked past me. â€Å"Adrian? You might want to keep your distance. A considerable distance.† Okay. Maybe a little scary. He obeyed and backed up. Ms. Terwilliger apparently had no such fear for herself because she stayed only a few feet away from me. â€Å"Now then,† she said. â€Å"Apply the ashes, and hold out your hand.† I reached into the bag, touching the ashes with my thumb and forefinger. Then I lightly rubbed all my fingers together until my whole palm had a fine gray coating on it. I set the bag down and then held out my hand in front of me, palm up. I knew what came next but waited for her instruction. â€Å"Summon your magic to call the flame back from the ashes. No incantation, just your will.† Magic surged within me. Calling an element from the world reminded me a little of what the Moroi did, which felt strange. My attempt started off as a red glimmer, hovering in the air above my palm. Slowly, it grew and grew until it was about the size of a tennis ball. The high of magic filled me. I held my breath, scarcely able to believe what I had just done. The red flames writhed and swirled, and although I could feel their heat, they didn’t burn me. Ms. Terwilliger gave a grunt that seemed to be equal parts amusement and surprise. â€Å"Remarkable. I forget sometimes what a natural you really are. It’s only red, but something tells me, it won’t take long before you can produce blue ones without the ashes. Calling elements out of the air is easier than trying to transform one substance into another.† I stared at the fireball, entranced, but soon found myself getting tired. The flames flickered, shrank, and then faded away altogether. â€Å"The sooner you get rid of it, the better,† she told me. â€Å"You’ll just use up your own energy trying to sustain it. Best to throw it at your adversary and quickly summon another. Try again, and this time, throw it.† I called the fire once more and felt a small bit of satisfaction at seeing it take on more of an orange hue. I’d learned in my very first childhood chemistry lessons that the lighter a flame was, the hotter it burned. Getting to blue anytime soon still seemed like a long shot. And speaking of long shots . . . I threw the fireball. Or, well, I tried. My control of it faltered when I attempted to send it off toward a bare patch of ground. The fireball splintered apart, the flames disappearing into smoke that was carried off by the wind. â€Å"It’s hard,† I said, knowing how lame that sounded. â€Å"Trying to hold it and throw it is just like an ordinary physical thing. I have to do that while still controlling the magic.† â€Å"Exactly.† Ms. Terwilliger seemed very pleased. â€Å"And that’s where the practice comes in.† Fortunately, it didn’t take too many attempts before I figured out how to make it all work together. Adrian cheered me on when I successfully managed to throw my first fireball, resulting in a beautiful shot that perfectly hit the rock I’d been aiming for. I flashed Ms. Terwilliger a triumphant look and waited for the next spell we’d be moving on to. To my surprise, she didn’t seem nearly as impressed as I expected her to be. â€Å"Do it again,† she said. â€Å"But I’ve got it down,† I protested. â€Å"We should try something else. I was reading the other part of the book – â€Å" â€Å"You have no business doing that yet,† she scolded. â€Å"You think this is exhausting? You’d pass out attempting one of the more advanced spells. Now.† She pointed at the hard desert floor. â€Å"Again.† I wanted to tell her that it was impossible for me not to read ahead in a book. It was just how I operated with all my classes. Something told me now was not the best time to bring that up. She made me practice the throw over and over. Once she was convinced I had it down, she had me work on increasing the fire’s heat. I finally managed to get up to yellow but could go no farther. Then I had to work on casting the spell without the ashes. Once I reached that milestone, it was back to practicing the throws. She picked various targets for me, and I hit them all effortlessly. â€Å"Just like Skee-Ball,† I muttered. â€Å"Easy and boring.† â€Å"Yes,† Ms. Terwilliger agreed. â€Å"It’s easy hitting inanimate objects. But moving targets? Living targets? Not quite so easy. So, let’s move on to that, shall we?† The fireball I’d been holding above my hand vanished as shock shattered my control. â€Å"What do you mean?† If she expected me to start aiming at birds or rodents, she was in for a rude awakening. There was no way I was going to incinerate something alive. â€Å"What am I supposed to hit?† Ms. Terwilliger pushed her glasses up her nose and backed up several feet. â€Å"Me.† I waited for the punch line or at least some further explanation, but none came. Go ahead, you can’t hurt me.† I had to think a few moments for how to phrase my next response. â€Å"I’m a pretty good shot, ma’am. I can hit you.† This earned an outright laugh. â€Å"Hit, yes. Hurt, no. Go ahead and throw. Our time is running out.† I didn’t know how much time had passed exactly, but the sun was definitely lower in the sky. I looked back at Adrian, silently asking for help in dealing with this insanity. His only response was a shrug. â€Å"You’re a witness to this,† I told him. â€Å"You heard her tell me to do it.† He nodded. â€Å"You’re totally blameless.† I took a deep breath and summoned my next fireball. I was so frazzled that it started off red, and I had to work to heat it up. Then I looked up at Ms. Terwilliger and braced myself for the shot. It was more difficult than I expected – and not just because I was worried about hurting her. Throwing something at the ground required almost no thought. The focus there was on aim and little else. But facing a person, seeing her eyes and the way her chest rose and fell while breathing . . . well, she was right. It was entirely different from hitting an inanimate object. I began to tremble, unsure if I could do it. â€Å"You’re wasting time,† she warned. â€Å"You’re sapping energy again. Throw.† The command in her voice jolted me to action. I threw. The fireball flew from my hand, straight at her – but it never made contact. I couldn’t believe my eyes. About a foot in front of her, it hit some kind of invisible barrier, smashing apart into small flames, which quickly dissipated into smoke. My jaw dropped. â€Å"What is that?† I exclaimed. â€Å"A very, very powerful shielding spell,† she said, clearly enjoying my reaction. She lifted up a pendant that had been hanging under her shirt. It didn’t look like anything special, just a piece of unpolished carnelian wrapped in silver wire. â€Å"It took incredible effort to make this . . . and requires more effort still in order to maintain it. The result is an invisible shield – as you can see – that’s impervious to most physical and magical attacks.† Adrian was by my side in a flash. â€Å"Hang on. There’s a spell that makes you invulnerable to everything, and you only now just thought to mention it? You’ve been going on this whole time about how Sydney’s in danger! Why don’t you just teach her this one? Then your sister can’t touch her.† Although it didn’t seem like Adrian was about to attack her as he had Marcus, he was almost just as upset. His face was flushed, his eyes hard. He had clenched his fists at his side, but I didn’t even think he noticed. It was more of that primal instinct. Ms. Terwilliger remained strong in the face of his outrage. â€Å"If it were that simple, then believe me, I would. Unfortunately, there are a number of problems. One is that Sydney, prodigy that she is, is nowhere near strong enough to cast this. I’m hardly strong enough. The other problem is that it has an extremely short time frame, which is why I’ve been so adamant about a schedule. It only lasts six hours and requires so much effort that you can’t just cast it and permanently keep it on you at all times. I’m already worn out and will be even more so once it fades. I won’t be able to cast it – or hardly any other magic – for at least another day. That’s why I need Sydney to be prepared at all times.† Neither Adrian nor I said anything right away. I’d taken note of her weary state when she got in the car but hadn’t thought much more about it. As we’d continued to practice out here, I’d observed her sweating and looking more fatigued, but I’d written it off to the heat. Only now could I fully appreciate the extent of what she had done. â€Å"Why would you go to so much effort?† I asked. â€Å"To keep you alive,† she snapped. â€Å"Now, don’t make this a waste. We’ve only got one more hour before it wears off, and you need to be able to aim at someone without thinking twice. You hesitate too much.† She was right. She even tried moving around a little, just to give me a better feel for what it’d be like with a real foe, but I didn’t find it to be much of a challenge. My shot went wide, the ball disintegrating before it got anywhere near her. The fire she’d released passed me, about a foot away. With a weary grin, she sank to her knees and exhaled. â€Å"Class dismissed,† she said. â€Å"What was that?† I asked. â€Å"I don’t have a magic shield on me!† She didn’t display my same concern. â€Å"It was nowhere near you. I made sure of that. It was simply to prove that no matter how ‘boring and easy’ this seems, all bets are off when someone is actually attacking you. Now then. Adrian, would you be kind enough to bring me my bag? I have some dried dates in there that I think both Sydney and I would appreciate right about now.† She was right. I’d been so caught up in the lesson that I hadn’t noticed how exhausted I had become. She was in worse shape, but the magic had definitely taken its toll on me. I’d never worked with amounts this big for so long, and my body felt weak and drained as the usual blood sugar drop occurred. I began to understand why she kept warning me away from the really difficult stuff. I practically inhaled the dried dates she’d brought for us, and although the sugar helped, I was desperate for more. Adrian gallantly helped us both walk back to the parking lot at the park’s entrance, keeping one of us on each arm. â€Å"Too bad we’re out in the middle of nowhere,† I grumbled, once we were all in Adrian’s car. â€Å"I think you’d be amazed at how much I could eat right now. I’ll probably faint before we’re back to some civilization and restaurants.† â€Å"Actually,† said Adrian. â€Å"You might be in luck. I think I saw a place not far from here when we were driving in.† I hadn’t noticed anything, but I’d been too preoccupied worrying about Ms. Terwilliger’s upcoming lesson. Five minutes after we were back on the highway, I saw that Adrian was right about a restaurant. He exited onto a drab little road, pulling into the gravel parking lot of a small but freshly painted white building. We were the only people under sixty in the whole place. We ordered our pie and sat down with it in a corner booth. I ordered peach, Adrian had French silk, and Ms. Terwilliger went with pecan. And of course, she and I had the waitress bring us coffee as soon as humanly possible since we’d had to abstain, painfully, for the magic. I took a sip and immediately felt better. Adrian ate his slice at a reasonable rate, like a normal person, but Ms. Terwilliger and I dug in as though we hadn’t eaten in a month. Conversation was irrelevant. Only pie mattered. Adrian regarded us both with delight and didn’t try to interrupt until we’d practically licked the plates clean. He nodded toward mine. â€Å"Another piece?† â€Å"I’ll take more coffee.† I eyed the sparkling plate and couldn’t help but notice that inner voice that used to nag me about calories was quiet these days. In fact, it didn’t seem to be around anymore at all. I’d been so angry about Adrian’s food â€Å"intervention,† but his words had ended up having a bigger impact than I’d expected. Not that it had anything to do with him personally, of course. Lightening up my dieting restrictions was just a reasonable idea. That was it. â€Å"I feel pretty good now.† â€Å"I’ll get you another cup,† he told me. When he returned, he even had a mug for Ms. Terwilliger. â€Å"Figured you’d want one too.† She smiled in appreciation. â€Å"Thank you. You’re very astute.† As she drank, I couldn’t help but notice she still looked tired, despite the fact that we’d just replenished with sugar. Facts and Characteristics of the Caspian Tiger One of three subspecies of Eurasian tiger to go extinct within the last century, the other two are the Bali Tiger and the Javan Tiger, the Caspian Tiger once roamed huge swaths of territory in central Asia, including Iran, Turkey, the Caucasus, and the -stan territories bordering Russia (Uzbekistan, Kazakhstan, etc.). An especially robust member of the Panthera tigris family, the largest males approached 500 pounds, the Caspian Tiger was hunted mercilessly during the late 19th and early 20th centuries, especially by the Russian government, which put a bounty on this beast in a heavy-handed effort to reclaim farmlands bordering the Caspian Sea. Why Did the Caspian Tiger Go Extinct? There are a few reasons, besides relentless hunting, why the Caspian Tiger went extinct. First, human civilization encroached mercilessly on the Caspian Tigers habitat, converting its lands into cotton fields and even looping roads and highways through it fragile habitat. Second, the Caspian Tiger succumbed to the gradual extinction of its favorite prey, wild pigs, which were also hunted by humans, as well as falling prey to various diseases and perishing in floods and forest fires (which grew more frequent with changes in the environment). And third, the Caspian Tiger was already pretty much on the brink, restricted to such a small range of territory, in such dwindling numbers, that virtually any change would have tipped it inexorably toward extinction. One of the odd things about the extinction of the Caspian Tiger is that it happened literally while the world was watching: various individuals were hunted died and were documented by naturalists, by the news media, and by the hunters themselves, in the course of the early 20th century. The list makes for depressing reading: Mosul, in what is now the country of Iraq, in 1887; the Caucasus Mountains, in the south of Russia, in 1922; Irans Golestan Province in 1953 (after which, too late, Iran made hunting the Caspian Tiger illegal); Turkmenistan, a Soviet republic, in 1954; and a small town in Turkey as late as 1970 (although this last sighting is poorly documented). Confirmed Sightings Although its widely considered to be an extinct species, there have been numerous, unconfirmed sightings of the Caspian Tiger over the past few decades. More encouragingly, genetic analysis has shown that the Caspian Tiger may have diverged from a population of (still extant) Siberian Tigers as recently as 100 years ago and that these two tiger subspecies may even have been one and the same animal. If this turns out to be the case, it may be possible to resurrect the Caspian Tiger by as simple an expedient as re-introducing the Siberian Tiger to its once-native lands of central Asia, a project that has been announced (but not yet fully implemented) by Russia and Iran, and which falls under the general category of de-extinction.

The narrative theory was seen to be applicable in the movie through the perspective of the main character, Chuck Nordstadt. At the beginning of the movie, Chuck was viewed to be narrating an incident where he was apparently at the center of the parade being honored for all the positive performance he exhibited. Apparently, Chuck was only dreaming. This is the first example from the movie where the narrative theory was clearly applied. Secondly, another story was delivered by Chuck’s sister, Gloria, who told Chuck the real story behind Chuck’s father’s death. Again, the narrative theory was evident since the actual story of what happened to Chuck’s father apparently happened years ago and without the knowledge of Chuck. Chuck’s mother and sisters deliberately withheld the information from Chuck. When Chuck learned about it, he was devastated. DOT offices are located in New Jersey Ave, SE Washington  District Of Columbia (DOT, 2012). This paper investigates the department of transportation on issues related to regulations and policy development. The paper also analyzes challenges and opportunities that define DOT operations. Purpose of the Agency The agency seeks to promote safety, efficiency, and convenience in transportation, in order to meet the interests of the Americans. Moreover, the agency promotes implementation of policies that concerns transport infrastructures and safety. The agency’s establishment was based on an economic agenda that sought to revitalize the US economy. According to Pucher, & Lewis (2000), the agency is an instrumental force in restructuring other economic factors such as land planning, energy conservation, utilization of scarce resources and urban planning. The agency also maintains equality in distribution of national resources, which characterize a free economy. In addition, DOT helps to create equal opportunity for all Americans through efficient transport system. The safety aspect enlisted in the agencies agenda also promotes economic prosperity. This is because the agency’s core agenda is to eliminate inconveniences associated with transport that often leads to losses (Bookman, 2010). According to Bookman (2010), the agency is consumer oriented. This is because the agency sought to promote the interests of ordinary Americans by promoting free flow of consumer goods. Evidently, the agency also has key interest energy and environmental conservation. The aim of the agency was to promote economic development through the creation of efficient transport infrastructures. These agendas are bound to misuse by businesses and individuals who are concerned with profits and gains. This created the need for regulations, which is the basis o f the agency’s operations. Indeed, the private sector views the agency as arm of the government established to oversees or promote transport regulations. DOT has various regulation options that were established to impose sanity in the transport sector. Regulation options promoted by the agency include driver regulations, vehicle regulations, company regulations, and alcohol and drug regulations (Bookman, 2010). Driver’s regulations ensure that drivers and the staff driving on American roads meet technical requirements. The regulations concerns licensing of drivers based on their qualification. The regulations also stipulate conditions under which a driver’s license may be revoked. On the other hand, vehicles regulatory options stipulates standard of vehicles used on American roads. Moreover, the regulations coincide with environmental conservation policies stipulated in DOT’s agenda. The alcohol and drug regulation outlines substance that should not be us ed by drivers. Alcohol and drug regulations promote compliance to drugs and substance regulations among drivers. The choice of these regulations is subject to review and amendment by the senate. DOT reviews the regulations to ensure that they meet the economic and social needs of the country. DOT’s executive board seems to have selected the most efficient regulations. This is because the regulations target road users and vehicles used American roads. Such regulations avoid redundancy and ensure accountability among road users. Cardiac Activity and Ventricular Tachycardia (VT) Waveform Lachlan Donnet-Jones Sudden cardiac death (SCD) is one of the leading causes of mortality in Australia. One of the primary causes of SCD is cardiac dysrhythmias, such as, Ventricular Tachycardia (VT). The most effective treatment for life-threatening cardiac dysrhythmias is defibrillation. This essay will examine the relationship between cardiac activity and the Ventricular Tachycardia (VT) waveform, and discuss how defibrillation may terminate this dysrhythmia, allowing the heart to return to a normal rhythm. The typical healthy adult heart will have a resting heart rate of between 60 and 100 beats per minute (Saladin, 2011). When the heart beats abnormally fast, it pumps less effectively, which decreases the level of perfusion to the tissue of the body, including the heart itself. This rapid heart rate increases the hearts muscle tissues (myocardium) demand for oxygen, and without intervention, can lead to the death of myocardial cells, which is known as a Myocardial Infarction (MI) (Huazers, 20). Each year in Australia approximately 55,000 people suffer a heart attack, or an Acute Myocardial Infarction (AMI). This is equal to 150 heart attacks per day or one in every 10 minutes (Heart Foundation). The Australian Bureau of Statistics reported that over 350,000 Australians will suffer an AMI at some point in their lives (ABS, health survey). In Trappes’ 2012 research article, Trappe notes that there is no single factor that causes an AMI, it is a multifactorial problem, however, approximately ninety percent of AMI’s are caused by tachyarrhythmia’s (Trappe, 2012). Before one can gain a thorough understanding of dysthymias, it is necessary to develop a fundamental grasp of the heart’s electrical conduction system and the associated physiology and pathophysiology. The primary function of the electrical conduction system is to transmit electrical impulses from the sinoatrial node (SA node) (normal site of conception) down to the atria and ventricles, triggering a contraction of heart muscle (myocardium) and controlling the heart rate. In a normal sinus rhythm, originating from the SA node, there are three phases; atrial depolarisation, ventricular depolarisation and atrial and ventricular repolarisation. The SA node is found within the wall of the right atrium proximal to the entrance of the superior vena cava. Similar to all electrical nodes within the heart, the SA node is composed of pacemaker cells which generate automatic and regular electrical impulses. These electrical impulses travel through the walls of the right atrium, causing contraction of the heart muscle (myocardium), to the atrioventricular node (AV node) via internodal conduction tracts (anterior, middle, and posterior). A final SA node conduction pathway, known as Bachmann’s bundle (interatrial conduction tract), transmits electrical impulses across the heart to the left atrium. On an electrocardiogram (ECG) this atrial depolarisation is represented by the P wave. The fibrous annulus is a non-conductive layer of tissue which prevents the electrical impulse from travelling outside the perimeter of the atrium. The primary function of the AV node is to process the electrical impulses from the atria to the bundle of His in a way that slows the impulses arrival at the ventricles by approximately 0.12 seconds. This delay allows for the atria to empty and the ventricles to fill before the next contraction. After the bundle of His, the electrical impulse will travel down the right bundle branch and the left common bundle branch. These bundle branches continue to subdivide into smaller branches, the smallest of which connect to the Purkinje network, an elaborate mesh of minute Purkinje fibres which spread throughout the ventricles. In a normal functioning heart it will take an electrical impulse approximately 0.2 seconds to travel from the SA node to the Purkinje network in the ventricles. On an ECG, this is shown as the P-R interval. At this point the impulse causes the ventricles to contract, pumping the blood out of the ventricles and into the systemic circulation. This depolarisation of the ventricles is represented by the QRS complex. Immediately following a QRS complex, is a period of time in which there is no electrical activity in the myocardium. This is known as the S-T segment and is normally represented as a flat line, level with the isoelectric line of an ECG. The proceeding T wave represents the repolarisation of the ventricles to their resting state. If at any point in this process the electrical impulse is disturbed, it can create a cardiac dysrhythmia, such as if the SA node were to produce rapid electrical impulses, resulting in tachycardia (fast heart beat). Ventricular Tachycardia (VT) is a type of tachycardia that originates within the inferior chambers of the heart, called the ventricles. The ventricles are the primary pumps of the heart, therefore, when they are compromised it can quickly deteriorate into a life-threatening dysrhythmia, such as, ventricular fibrillation (VF) or asystole (Chou, 2008). The diagnosis of VT is made by examining the rhythm seen on a 12-lead electrocardiogram (ECG). Although numerous diagnostic criteria have been developed, such as the ‘Brugada Criteria’ (Brugada, 1991), the following are the most commonly accepted (Riley, 2008). The rate of VT is above 100 per minute, typically 150 to 200, with a regular rhythm. The R-S complex is absent in precordial leads, and there are three or more consecutive Premature Ventricular Contractions (PVCs) present (AV dissociation). The ectopic pacemaker is below the Atrioventricular node (AV node), therefore, the PR interval is irrelevant. In addition, different ambulance services will have their own specific diagnostic criteria for VT, for example, Ambulance Tasmania (AT) Clinical Practice Guidelines (CPG’s) state that the rhythm must present with QRS complexes of over 0.12 seconds, and be sustained for a period of over 30 seconds (sustained VT). VT can be classified using three methods; morphology, episode duration, and symptoms. In regards to morphology, there are two primary categories of VT; monomorphic and polymorphic. Monomorphic VT has numerous causes, but is determined by consistent appearance across all leads of an ECG. A common reason that the beats from each lead appear the same, is because the impulse is being generated from an increased rate of automaticity in a single point from the left or right ventricles. This means that the pacemaker cells, such as the Purkinje fibres in the left and right ventricles, that are able to reach an action potential on their own accord (automaticity), have increased the rate at which they fire impulses (intrinsic rate). Another reason for monomorphic VT is due to the presence of a re-entry circuit within the ventricle. A re-entry circuit occurs when an electrical impulse constantly travels in a constricted circle within the heart, as opposed to moving from one end of the heart to the other, like a normal electrical impulse circuit. Although monomorphic VT has many causes and contributing factors, the most common cause is scarring of the myocardial tissue from a previous MI episode. The scarred tissue left behind does not conduct electrical impulses, and therefore, the potential for a circuit around the scar can result in tachycardia. This is similar to the aforementioned re-entrant circuit, and is a common cause of other dysrhythmias, such as, atrial flutter (Af) and supraventricular tachycardia (SVT). Scar-related monomorphic VT is predominantly prevalent in patients who have a previously survived a MI, particularly in those who have damaged myocardium as a result (John, reference). Unlike the cons istent rhythm seen is monomorphic VT, polymorphic VT is an irregular rhythm that has constant variations in its morphology. A second method to define VT is studying the duration of the episode. Three or more consistent contractions on an ECG, originating from within a ventricle at over 100 beats per minute, is determined as VT. If the tachycardia rhythm terminates itself in under 30 seconds, it is considered non-sustained VT. If the rhythm continues beyond 30 seconds, it is considered sustained VT. The final method to classify VT is reviewing symptoms. When a patient is in VT, the loss of co-ordinated atrial contraction and high heart rate can impair cardiac output (CO), and therefore, they will not have a palpable pulse. This is known as Pulseless VT. Pulseless VT is concomitant with an absence of cardiac output (CO), and therefore, according to AT clinical practice guidelines, is to be treated as worst case scenario, which is ventricular fibrillation (VF), a shockable rhythm (CPG Reference). In a report from the American College of Cardiology, Zipes et. al note that VT can occasionally be accompanied by reasonable cardiac output and may even present as asymptomatic, however, the heart will not tolerate this rhythm for a sustained period of time, and will eventually deteriorate to pulseless VT or VF. Supraventricular Tachycardia (SVT) with a bundle branch block (BBB) or Wolff-Parkinson-White syndrome is commonly misdiagnosed as VT (Trappe). This is due to the similar diagnostic characteristics, such as, wide QRS complexes and high heart rates, which are mutual in all wide complex tachycardia (litfl). It is important to differentiate the two because certain medications used to treat SVT could potentially worsen the patient’s condition. As Trappe notes in his research article ‘Treating critical supraventricular and ventricular arrhythmias’, it is always beneficial to treat for the worst case scenario, in this case, VT (Trappe, 2010). This opinion is mutual in regards to Ambulance Tasmania CPG’s, where it recommends treating for worst case scenario. Once a shockable dysrhythmia has been recognised, it is necessary to intervene with an external source of electrical activity to correct the hearts rhythm. Defibrillation is the standard and most effective treatment for cardiac dysrhythmias, such as VT and VF (Reference). Defibrillation is the process of using a device called a defibrillator to deliver a therapeutic measure or ‘shock’ of electrical current through the heart. The current delivered, aims to depolarise a critical mass (Critical mass theory**) of the heart muscle (myocardium), interrupting the dysrhythmia and allowing the heart’s natural pacemaker, the SA node, to return to a normal sinus rhythm. Defibrillators are becoming widely available in the form of transvenous, implanted (implantable cardioverter-defibrillator), or external (automated external defibrillators) devices. Despite the different forms a defibrillation device may present in, they all operate on the same principle. There are two different methods of delivering an electrical shock from a defibrillation device; monophasic and biphasic waveforms. Monophasic is the ‘old’ method in which the electrical current travelled in one direction through a patient’s chest. The second method is using a biphasic waveform, meaning the current is delivered to the heart in two vectors (two directions). Due to the use of two vectors, the peak electrical current needed to revert a dysrhythmia is decreased to 200 joules, as opposed to 360 joules of a monophasic waveform. Due to the high voltage (360 joules) used in monophasic waveform it can cause superficial burns to the patients skin. Additionally, _____ found the use of a biphasic waveform to be more effective at returning the heart to a sinus rhythm and resulted in less damage to myocardium, leading to better patient outcomes (Reference) . ____ notes that for the aforementioned reasons, monophasic waveform defibrillation is quickly being replaced with biphasic (Reference). Violent Video Games: A Bad Choice for Parents and Children A twelve year old boy named Paul sits about three feet from the fifty-two inch screen television with his eyes fixated on his character, a humanlike fox. He is able to use his game controller without looking away from the television. He sits erect and is so focused on what he is doing that he is unaware of everything that is going on around him. Occasionally he will yell out "Die! Die, sucker!" Just then his five year old brother, Skyler, stops beside him. Skyler exclaims with a smile, "What are you playing?" followed by, "Can I play?" The violence has caught his eye. Paul replies, "Sure, but I need to show you how to work the controller." He continues explaining, "This is Star Fox Adventure and see this red thing moving. That is your fox's sight box. What you see in the box is what the fox sees. You aim it at things and you push this 'A' button to make the fox shoot rockets at things and blow them up." Skyler begins to get really excited, laughing and shouting, "T hat's cool!" It did not take Skyler very long to become engulfed by Paul's Nintendo GameCube system to the point of being totally unaware of all the other children running around him. This scene quickly brought to my mind a question: Are video game systems good or bad for children within these age groups? A parent should get educated on this issue before choosing to buy one of many video systems. Parents should be concerned about the effects on their children.
